What is Formstack used for?

Formstack is used for data collection and the business processes that follow it. Typical examples are client intake and onboarding, applications and registrations, order forms with payments, event registrations, HR requests with approval workflows, surveys and market research, and patient or student forms in healthcare and education. Marketing teams also use it for lead capture on their website and for market research, because every submission can go straight into the CRM, where marketing and sales both see it. Teams build forms quickly with drag and drop, route each submission to the right people, and send the form data to the business tools they already rely on, such as Salesforce, HubSpot or a spreadsheet.

The difference from a simple form builder is what happens after the submission. Formstack can create a contract, invoice or letter from the collected data, send it for signature and store the result, so one step replaces several manual ones. That is why organizations in regulated fields, such as healthcare, finance, insurance and government, use it to reduce errors and save time on paperwork. It is also a common choice for education services such as admissions and financial aid.

  • Salesforce-native versions: Formstack for Salesforce runs inside Salesforce and writes to Salesforce objects directly (AppExchange listing).
  • Security controls: data encryption, access control per individual user and features for compliance requirements such as HIPAA, PCI and GDPR.
  • Reports and analytics: see submission counts, drop-off and partial submissions, and export data to spreadsheets or other services.

Formstack pricing

Formstack sells Forms on its own and Documents and Sign only as part of the Suite. The plans differ in features, builders and limits, so it is important to compare them against your processes. The plans below come from the Formstack pricing page as of September 2026.

  • Forms: $83 per month billed annually, or $99 month to month. One builder user, 25 forms, 1,000 submissions per form and 2 GB of storage.
  • Suite (Forms + Documents + Sign): $250 per month billed annually, or $299 month to month. Three builder users, 100 forms, 5,000 submissions per form, 10 GB of storage, 250 document deliveries a month and unlimited eSignatures.
  • Enterprise and Salesforce plans: custom pricing. Enterprise adds a success manager, SSO, unlimited users and submissions and API access.

There is no free plan. Formstack offers a 14-day free trial of Forms with no credit card required.

The cost surprise to check is how the limits map to your use. The Forms plan counts builder users, forms and submissions per form, so a team that needs a second builder or more than 25 forms moves to the Suite or Enterprise. Documents and Sign can't be bought separately. Compliance needs, such as HIPAA with a business associate agreement, are usually handled on higher plans, so confirm with sales before you commit.

Formstack Forms, Documents and Sign: from data collection to signature

The three products are designed to work together. Forms collects the data. Documents turns it into a finished file: for example, an insurance application becomes a filled-in policy PDF, or a sales order becomes a contract with the customer's details. Sign then sends that file for signature and returns the signed copy with an audit trail. Each product also works on its own inside the Suite, so a team can generate files from Salesforce records without a form, or send any PDF for signature. Because the three parts are integrated, the same data is typed once, which cuts errors.

For low-code teams, this chain replaces a stack of separate tools and simplifies the change from paper to digital processes. The trade-off is that each step is a Formstack product, so switching one part later means rebuilding the connection.

Compliance requirements and security

Formstack is often chosen for its compliance features. It supports HIPAA, PCI and GDPR requirements with data encryption, audit trails and access control, and it publishes a HIPAA page for Formstack Sign. Healthcare accounts limit integrations to services that meet the same data-protection standards. Formstack is cloud only; there is no self-hosted version.

The drag-and-drop editor is intuitive for simple forms; conditional logic, calculations and approval steps take longer to learn. Connect the business tools the data has to reach before you share the form, test a few submissions and check what each individual user can see. Decide on a paid plan only once you understand how many builders, forms and submissions you need, because those limits set the price. Many teams create and publish a first form quickly, often the same day, and discover the more advanced features later.
